Executive summary

Conversational AI systems are no longer passive tools; they are companions, counselors, and confidants. This deep integration into human emotional life brings new psychological risks. This whitepaper introduces the concept of AI Psychosis, a state where prolonged AI interaction contributes to or intensifies delusional thinking, mania, or self-harming behavior. Across continents, the same pattern repeats: prolonged AI conversations, emotional dependency, delusional reinforcement, and tragic outcomes. These are not isolated software errors, they are governance failures.

  1. Emotional dependency: Users anthropomorphize AI, treating it as a confidant. The absence of reciprocal human cues deepens dependency.

  2. Delusional reinforcement: AI models mirror users’ emotional states and beliefs, unintentionally validating distorted thinking.

  3. Crisis escalation gaps: Many AI systems fail to escalate when users show signs of distress, instead maintaining harmful dialogue loops.

  4. Cognitive overload: Continuous interaction and sleep disruption lead to emotional dysregulation and detachment from reality.
